Treatment of brain metastases has evolved significantly — stereotactic radiosurgery (SRS) has largely replaced whole-brain radiation therapy (WBRT) for limited brain metastases due to superior neurocognitive preservation. Checkpoint immunotherapy has demonstrated intracranial activity in melanoma and NSCLC brain metastases, and targeted therapies (tucatinib, osimertinib) have demonstrated CNS activity in HER2+ and EGFR-mutant brain metastases — transforming the treatment of this common complication of cancer.
