Bernard Fisher transformed breast cancer surgery by proving through the NSABP B-06 trial that lumpectomy with radiation was equivalent to radical mastectomy for survival, establishing breast-conserving surgery as standard care. His paradigm shift demonstrated breast cancer was a systemic disease from inception, fundamentally changing surgical philosophy. As NSABP chairman, he built the infrastructure for modern cooperative group oncology trials.
