Alberto Bardelli has been a pioneer in using ctDNA to track clonal evolution of resistance to anti-EGFR therapy in colorectal cancer in real time, demonstrating that RAS mutations emerge in ctDNA months before radiographic progression and that their subsequent decline can guide rechallenge therapy strategies. His research at the University of Turin has established the principle that ctDNA monitoring of resistance mutations can inform treatment cycling and rechallenge decisions, creating a paradigm for evolutionary-guided therapy. His work has transformed the management of EGFR-targeted therapy in colorectal cancer through liquid biopsy monitoring.
