👥Biography 个人简介
Heidi Nelson, MD is the Fred C. Andersen Professor of Surgery and Chair of the Division of Colon and Rectal Surgery at Mayo Clinic, where she has led one of North America's most productive colorectal surgical oncology programs for more than two decades. She is the principal investigator of the landmark Clinical Outcomes of Surgical Therapy (COST) trial — the first definitive North American randomized controlled trial demonstrating that laparoscopic colectomy for colon cancer produces equivalent long-term cancer outcomes with superior short-term recovery compared with open resection, changing the standard of care worldwide. Beyond the COST trial, Dr. Nelson has led large-scale investigations into robotic-assisted rectal surgery, including her role in the ROLARR trial comparisons and subsequent multi-institutional robotic rectal resection registries. Her work on total mesorectal excision quality, surgical margin assessment, and standardization of rectal cancer operative technique has elevated surgical precision standards nationally. She has also driven important research on colorectal cancer screening, hereditary colorectal cancer syndromes, and the integration of enhanced recovery pathways (ERAS) in colorectal oncology. Dr. Nelson served as President of the American Society of Colon and Rectal Surgeons (ASCRS) and the Society of Surgical Oncology (SSO) and is a member of the National Academy of Medicine. She has received every major honor in colorectal and general surgical oncology and has trained dozens of academic colorectal surgeons across North America.
🧪Research Fields 研究领域
🎓Key Contributions 主要贡献
COST Trial — Laparoscopic vs Open Colectomy for Colon Cancer
Led the multicenter COST phase III RCT (n=872) demonstrating equivalent 3-year recurrence-free and overall survival between laparoscopic-assisted and open colectomy for colon cancer, while confirming laparoscopic advantages in recovery and hospital stay, establishing minimally invasive colectomy as an oncologically safe standard.
Robotic Rectal Resection Standardization
Pioneered standardized robotic total mesorectal excision (TME) technique and led multi-institutional outcome registries demonstrating equivalent oncologic outcomes with improved conversion rates and autonomic nerve preservation compared with laparoscopic TME, particularly for low rectal tumors in narrow pelves.
Surgical Margin Quality in Rectal Cancer
Produced landmark analyses of circumferential resection margin (CRM) involvement as a primary predictor of local recurrence and survival after rectal cancer surgery, contributing to standardized pathologic assessment protocols adopted in national and international rectal cancer guidelines.
Representative Works 代表性著作
Comparison of Laparoscopically Assisted and Open Colectomy for Colon Cancer (COST Trial): Long-Term Outcomes
New England Journal of Medicine (2004)
Landmark phase III RCT establishing equivalent overall and disease-free survival between laparoscopic and open colectomy for colon cancer, with the laparoscopic approach offering superior perioperative outcomes — the definitive evidence driving adoption of minimally invasive colectomy as standard practice.
Robotic Versus Laparoscopic Rectal Resection for Rectal Cancer: Updated Multi-Institutional Analysis
Annals of Surgery (2024)
Propensity-matched multicenter analysis confirming equivalent 3-year DFS and OS with robotic TME, with lower conversion rates and comparable rates of positive CRM, supporting robotic rectal surgery adoption.
