Alessandro Gronchi is the director of surgery at the Istituto Nazionale dei Tumori in Milan and the world's foremost authority on retroperitoneal sarcoma surgery. He founded and co-chairs the TransAtlantic Retroperitoneal Sarcoma Working Group (TARPSWG), which has produced landmark consensus guidelines and prospective data redefining surgical strategy for this challenging disease. His research has clarified the role of extended vs. standard resection for retroperitoneal liposarcoma and established the critical importance of histotype-tailored surgical approaches.

Extended vs. Standard Resection for Retroperitoneal Liposarcoma
Led the STRASS randomized phase III trial comparing extended resection with standard resection for primary retroperitoneal sarcoma, providing the only prospective randomized data in this disease and clarifying the marginal benefit of extended compartmental surgery specifically in dedifferentiated liposarcoma.
Histotype-Tailored Surgery and Nomogram Development
Developed and externally validated the Memorial Sloan Kettering/INT-Milan sarcoma nomograms predicting recurrence and survival after resection of retroperitoneal sarcoma, enabling individualized counseling and decision-making for patients and clinicians.
